Thad Ziemba, CPCU is your State Farm independent agent, offering you Medicare Supplement and Long-Term Care insurance, among other plans, because we care.

Here is information about two of our popular plans:

State Farm Medicare Supplement Insurance
Just about everyone agrees Medicare is one of the most important programs available to senior citizens. But, Medicare does not pay for everything. And, for some people, this is a severe strain on retirement finances. State Farm's Medicare Supplement insurance can help pay many hospital, medical, and surgical expenses Medicare does not cover.

A few aspects of our coverage:
- There are no exclusions for preexisting conditions.
- When Medicare deductibles increase, State Farm's benefits increase (Plans B, C and F)
- State Farm will not cancel or refuse to renew your policy because of poor health or because you have had too many claims.
- Premiums for your policy cannot be increased unless they are increased for all other policies like yours.

There are exclusions and limitations to this policy, which meets the minimum standards for Medicare Supplement Insurance as defined by the New York State Insurance Department.

State Farm Long-term Care Policy
At State Farm we know that maintaining your independence and quality of life is important to you, so we have designed our Long-term Care policy to do everything possible to help you get the level of care you need in the setting YOU prefer.

With State Farm's Long-term Care policy, you can:
- Choose home health care
- Select the provider or facility of your choice
- Preserve your retirement estate
- Avoid dependence on others.

Our policy is tax-qualified, which means your premiums qualify as a medical expense for income tax purposes.

State Farm's Long-term Care policy provides long-term care facility and home or community-based care benefits if you become chronically ill. The options you select gives you a trust "fund" of money from which you can draw benefits. As long as your claim is active, benefits are yours to the last dollar.

There are exclusions and limitations to this policy. If any provision of this policy conflicts with the laws of the state where you live on the effective date of coverage, the provision is automatically changed so as to comply with the minimum requirements of the state.

Visit State Farm's Web site for more information on these and other sensible, affordable protection plans, or call Thad Ziemba at 315.737.7359.
This information provides a glimpse of coverage and is not a contract. Contact us for exclusions and limitations. Modifications are applicable in some states.
